29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) On or before 2/3/2023 the employer did not ensure that forklifts were taken out of service and repaired before being placed back into service in that a front headlight on a forklift used in the warehouse was not functioning. This condition exposed employees to struck-by hazards. (b) On or before 2/3/2023 the employer did not ensure that forklifts were taken out of service and repaired before being placed back into service in that a blue warning light on top of a forklift used in the warehouse was not functioning. This condition exposed employees to struck-by hazards.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place: (a) On or before 2/3/2023 the warning/caution labels and nameplates on forklift 217, 2017 Toyota 8FGU25, Serial #79037 were damaged and unreadable. Some of the required labels were missing as well. (b) On or before 2/3/2023 the warning/caution labels and nameplates on forklift 119, 2019 Toyota 8FGU25, Serial #94108 were damaged and unreadable. Some of the required labels were missing as well.
