29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  On or about February 6, 2023, and at times prior thereto, at the residential construction site, employees were not wearing safety glasses while operating a nail gun to install shingles on the roof of a one-story building. The employees were potentially exposed to struck-by and flying particles hazards.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about February 6, 2023, and at times prior thereto, at the residential construction site, the employer did not ensure employees installing shingles on the roof of a one-story building wore fall protection at approximately nine (9) feet or more above the lower levels, exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Ladder extension was as such that ladder deflection under a load, did by itself, cause the ladder to slip off its support:  On or about February 6, 2023, and at times prior thereto, at the residential construction site, the employer did not ensure employees were using the ladder side rails extended at least three (3) feet above the upper landing surface to access the roof, exposing employees to fall hazards (approximately nine (9) feet or more above the lower level).
