29 CFR 1910.179(k)(1)(ii): The actuating mechanism of the limit switch was not located so that it would trip the switch, under all conditions, in sufficient time to prevent contact of the hook or hook block with any part of the trolley:  (a) On or before 2/2/23, the employer did not ensure that the limit switch would arrest the upward motion of the lower block on the auxiliary hoist of the Cleveland bridge crane (sn: 18160-82) to prevent the lower block from coming into contact with the trolley. The lower block of the auxiliary hoist traveled past the limit switch and contacted the trolley, breaking the rope, causing the block to fall. This condition exposed employees and vendor employees to falling crane blocks.
