29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. On or about February 7, 2023, at the establishment located at 240 Otter Falls Trail Marathon, Wisconsin; employees were exposed to caught-between and crushed-by hazards while moving stored wire rolls from the storage ramp to the Mid-America Manufacturing W14-PLC Wire Roller machine (SERIAL #95-W14-08-WMS-209). Employees caught between the wire roll and machine carriage could suffer lacerations, contusions, and broken bones.

On or about February 7, 2023, at the establishment located at 240 Otter Falls Trail Marathon, Wisconsin; the employer did not ensure that the nameplate of the Clark CY160 forklift was in place and legible.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.) On or about February 7, 2023, at the establishment located at 240 Otter Falls Trail Marathon, Wisconsin; the Milwaukee 5040 pedestal grinder (S/N 6000064KOH2), near the Plant 1 office, did not have an adjustable tongue guard, exposing employees to struck by hazards.
