29 CFR 1926.1053(b)(1): Where (a) portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, or the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about February 3, 2023, on the southside of the building, employees were exposed to a fall hazard while using a ladder that was not secured at the top and provided with a grasping device.

On or about February 3, 2023, on the south side of a building, employees were exposed to a fall hazard greater than 15 feet while welding structural steel on the roof without being protected by a fall protection system.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. On or about February 3, 2023, on the southwest side of a building, two employees were exposed to a fall hazard while welding angle iron onto a steel beam from an aerial lift without wearing a harness with a lanyard attached to the boom or basket.
