On a roof, at a temporary residential construction site, located at 3500 University Boulevard, in Jacksonville FL, at building 10: On or about February 7, 2023, 3MG Solutions dba 3MG Roofing, failed to ensure that its subcontractor Knights Roofing & restorations, protected its employees from fall hazards, in that fall protection was not utilized by employees while performing roofing operations. 3 GM Solutions LLC dba 3MG Roofing, was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1561377, citation number 1, item number 1, and was affirmed as final order on December 21, 2021, with respect to workplace located at 564 Marquesa Circle, SAINT JOHNS, FL 32259.

At a temporary multi-family residential construction site, located at 3500 University Boulevard, in Jacksonville FL, at building 10: On or about February 7, 2023, 3MG Solutions dba 3MG Roofing, failed to ensure that its subcontractor Knights Roofing & Restorations, protected its employees from fall hazards, in that the ladder rails did not extend 3 feet above the upper landing surface of the roof at the eave.
