29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a.	On a 5/12 pitched residential roof located at 1610 Fort Smith Boulevard, in Deltona Florida: On or about March 06, 2023, the employer exposed employees to fall hazards ranging from 6 to 14-foot, in that fall protection was not utilized by employees, while engaged in residential construction activities.  3MG Soultions, LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1561377 Citation number 1, Item number 1 and was affirmed as a final order on 12/21/2021 with respect to a workplace located at 564 Marquesa Circle, Saint Johns, Florida 32259.

29 CFR  1904.41(a)(2):29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  a) 3MG Solutions, LLC located at 1127 Solana Avenue, Winter Park Florida 32789: On or about March 14, and at times prior, the employer failed during calendar year 2023, to electronically submit information from their OSHA Form 300A or equivalent by 03/02/2023.  The establishment employed approximately 77 employees and was classified in the North American Industrial Classification System as 238160.
