29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about 02/07/2023 at a residential construction job site located at (Celtic Meadows) Miremont Drive - Lot 3, Manchester, MO., Lampkin Masonry employees were installing masonry materials to the side of a structure while being positioned on a supported scaffold. The top platform had several stacks of masonry bricks along with other job-related materials staged on a plank. Exposed employees working directly underneath the masonry materials were not wearing protective helmets.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  On or about 02/07/2023 at a residential construction job site located at (Celtic Meadows) Miremont Drive - Lot 3, Manchester, MO., Lampkin Masonry employees were installing masonry materials to the side of a structure while being positioned on a supported scaffold. One employee gained access to the top plank by means of using cross braces.

On or about 02/07/2023 at a residential construction job site located at (Celtic Meadows) Miremont Drive - Lot 3, Manchester, MO., Lampkin Masonry employees were installing masonry materials to the side of a structure while being positioned on a supported scaffold approximately 18 feet above ground, which is the next lower landing surface.

Protective guard rails were not installed on the scaffold.
