29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. On or about 10/03/2024 at a residential construction job site located at 330 Creek Sound Ct. in Wentzville, MO., Lampkin Masonry employees were installing brick facade to the side of a structure while being positioned on a supported scaffold approximately 14 feet above ground. The employer did not install guys, ties and/or braces at all on a scaffold to prevent swaying that was approximately five (5) feet wide and over twenty (20) feet high.

On or about 10/03/2024 at a residential construction job site located at 330 Creek Sound Ct. in Wentzville, MO., Lampkin Masonry employees were installing brick facade to the side of a structure while being positioned on a supported scaffold approximately 14 feet above ground, which is the next lower landing surface.

29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph. On or about 10/03/2024 at a residential construction job site located at 330 Creek Sound Ct. in Wentzville, MO., Lampkin Masonry employees were installing masonry materials to the side of a structure while being positioned on a supported scaffold. Protective guard rails were not installed on the scaffold and the employees were exposed to fall hazards.
