Employees were exposed to point of operation and flying debris hazards.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Maintenance Squadron, Fabrication Flight ATM Shop: The plug end of the commercial pedestal fan (brand Air Master Fans) electrical cord was missing its grounding prong. Employees were exposed to electrical hazards.
