29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface that have side rails that extend at least 3 feet (0.9 m) above the upper landing surface.  (a) On or about February 6, 2023, the employer did not ensure employees could reach upper landing on press five (5) while using an A-frame ladder that did not extend three (3) feet above the landing surface. This exposed the employee to a fall hazard of eight (8) feet.

29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  (a) On or about February 8, 2023, the employer did not provide training to employees on how to implement personal fall protection systems when performing work  that exposes employees to fall hazards at height above four (4) feet or hazardous processes. Hazardous processes such as performing work onto of press five (5) and using man lifts.

29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) On or about February 8, 2023, the employer did not develop and utilize machine specific hazardous energy control procedures for press five (5) and associated equipment (shuttle system, motor, and garage door),  to isolate the different energy sources. This exposed maintenance employees and press operators to caught and crushing hazards and amputation hazard.
