29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition. In the Bulk Room, the bulk system mixer had combustible dust greater then 1.5 inches in depth. Employees were exposed to combustible dust hazard that can cause catastrophic destruction, injuries and deaths.

29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation: Employees were exposed to electrical shock hazards in the combustible dust bulk room due to extension cord insultation being pulled away exposing internal wires from not having proper strain relief. The extension cord was in use and was powering the wall fan that was in operation at time of inspection.
