29 CFR 1910.147(a)(3)(i): The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees. At 1400 E. Logan Avenue, Emporia KS, employees were exposed to struck by hazards in that the employer did not provide documented procedures in disassembly procedures for the Emporia Manin Plant Meat Room Line #5 Scan Steel-Food Tech Model 550 Grinder.

29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: At 1400 E. Logan Avenue, Emporia KS, employees were exposed to struck by hazards that the employer did not provide procedures and training in procedures to ensure the clamp ring chain was connected to the hoist thereby isolating the struck by energy that was stored in the clamp ring for Emporia Main Plant Meat Room Line #5 Scan Steel-Food Tech Model 550 Grinder.
