Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On February 8, 2023, Revoultionarybuilders L.L.C. was framing a residential building located at 331 Dairiada Boulevard in O'Fallon, Missouri. The foreman and an employee were installing roof trusses, approximately 21 feet above the ground, while standing on the top plate. They were not utilizing guardrail systems, a safety net system, or a personal fall arrest system, exposing them to fall hazards.
