29 CFR  1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4):  a) On or about March 1, 2023, in the Raw Department, employees required to service and maintain equipment during disassembly and cleaning process on the stuffer machines, were not performing group lock out while performing such duties, thereby exposing employees to amputation hazards.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) On or about March 1, 2023, in the Raw Department,  employees operating electrical walkies to move pallets were not provided with powered industrial vehicle training prior to operating equipment, thereby exposing employees to struck-by hazards.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Packaging Department - On or about February 1, 2023, the employer failed to provide effective guarding to protect employees working at or near the Crumble 180A Mixer Grinder.  An employee's fingertip was amputated due to not having the exit end of the grinder guarded, thereby exposing employees to amputation hazards.   b) Raw Department -  On or about March 1, 2023,  the employer failed to provide effective guarding to protect employees working at the exit end of the AMFEC Mixer where employees were exposed to rotating paddles during operation, thereby exposing employees to amputation hazards.
