This hazard exposed employees to crushing and amputation hazards.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles, to protect them from the rays when noncombustible or flameproof screens or shields were not used: (a)Trail-Eze Inc. at 27152 U.S. Highway 281, Corsica, SD 57328: On or about and at times prior to February 14, 2023, multiple areas of the plant, including designated common travel areas were exposed to welding flash due to welding curtains not surrounding welding operations. No eye protection for exposure to welding flash was required or provided for non-welding exposed employees. This condition exposed employees to temporary impairment of vision, temporary blindness and headaches associated with welding flash exposure.

29 CFR 1910.305(g)(1)(iv):Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv): (a)Trail-Eze Inc. at 27152 U.S. Highway 281, Corsica, SD 57328: On or about and at times prior to February 14, 2023, the employer used flexible cords and/or cables as a substitute for the fixed wiring in that temporary power taps and extensions cords were being used to power the Robo Vent air filtration system installed outside of the north side of the building for periods greater than 90 days. This condition exposed employees to electrical shock and fire hazards. (b)Trail-Eze Inc. at 27152 U.S. Highway 281, Corsica, SD 57328: On or about and at times prior to February 14, 2023, the employer did not ensure temporary flexible electrical cords were not run through openings in walls of the production facility. Extension cords were identified as traveling through walls to power the Robo Vent air filtration system installed outside of the north side of the building. This condition exposed employees to fire and shock hazards.
