29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about February 7, 2023, the employer's hazardous energy control procedures, such as, but not limited to: 40163-0008, 40163-0012, 40163-0011, 40163-0041, 40163-0010, 40163-0001, 40163-0009, 40163-0005, 40163-0037, 40163-0060, 40163-0036, 40163-0061, 40163-0035, 40163-0065, 40163-0007,40163-0006,40163-0034, 40163-0062, 40163-0003, 40163-0002, 40163-0038,40163-0000, & 40163-0076 did not contain the required information. A. 29 CFR 1910.147(c)(4)(ii)(B), Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: The procedures did not provide steps for shutting down the machines nor did they identify all sources of hazardous energy, such as air, steam, and hydraulic energy sources. B. 1910.147(c)(4)(ii)(D), Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: The procedures did not address specific actions to perform for verification of the effectiveness of the lockout implementation and other energy control measures.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): On or about February 7, 2023, employees were exposed to machine hazards associated with moving parts, in-going nip points during cleaning activities on the Langston Single Facer C Flute machine, serial number 803422. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work, per the 1910.147(d)(2) requirements. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
