On or about 25 June 2025, employees of Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, were exposed to fall hazards. Employees were using a 6-foot A-frame ladder to access a space within Sugar Coat 1 Oven that was over 6 feet high.

On or about 25 June 2025, Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, failed to establish emergency response and rescue procedures prior to entry into a known permit-required confined space. An employee entered Sugar Coat 1 Oven to perform scheduled cleaning:  a. The employer failed to prevent unauthorized personnel rescue attempts when an employee entered a confined permit space following an incident to attempt rescue; b. The employer failed to verify the ability of the local fire department to perform permit required confined space rescue in the facility's approximately 103 known permit required confined spaces.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished.  On or about 25 June 2025, Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, failed to verify equipment was isolated and de-energized prior to the start of maintenance activities. The employer failed to verify equipment de-energization after power to the operating panel was turned off. The lack of verification exposed employees to amputation and crushing hazards while they worked inside and around the Sugar Coat 1 Oven.

On or about 25 June 2025, employees of Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, were exposed to mechanical, atmospheric, and thermal hazards while working on or inside an industrial oven, Sugar Coat 1 Oven, due to inadequate energy procedures.  a. A specific statement of the intended use of the procedure; b. Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; c. Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and d. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  Gilster-Mary Lee Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1650556, citation number 1, item number 1 and was affirmed as a final order on 25 August 2023, with respect to a workplace located at 2301 S Wabash Avenue, Centralia, IL 62801.
