29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented, and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. On or about February 8, 2023, and at times prior, the employer did not develop and document an equipment specific lockout/tagout procedure for employees to utilize while performing maintenance and repair work on vertical trash compactors. On February 8, 2023, an employee's hand was caught after being exposed to gravitational energy (mechanical) hazards caused by a hinged ram that was not supported while removing a hydraulic cylinder.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a. On or about February 8, 2023, and at times prior, employees were performing service and maintenance work on trash compactor equipment. The employer did not ensure that each employee received authorized lockout/tagout training to ensure that employees could recognize the hazardous energy sources present in the workplace and the methods necessary to ensure that hazardous energy sources are effectively isolated and controlled.
