29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  (a) On or about February 14, 2023, the employer did not ensure a competent person was conducting frequent and regular inspections of the job site to ensure fall protection was being worn, fall protection systems were installed properly, ladders were properly maintained, and job site conditions were maintained safe.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about February 14, 2023,  employees were operating pneumatic nail guns without eye  protection while installing shingles. This exposed employees to  eye injury.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about February 14, 2023, employees conducting roofing activities (shingle installation) on a 6:12 pitch roof of the main house structure were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately ten (10) feet five (5) inches on the South and East sides of the structure.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about February 14, 2023, the portable extension ladder being used to access the upper landing area or roof of the structure did not extend at least three feet above the upper landing area. This exposed employees accessing and egressing to and from the roof to a fall hazard of approximately Ten (10) feet, Five (5) inches.
