On the second tap, flash fragments discharged from the forge. The employee sustained a puncture wound through the abdomen. b) Employee performing forging activities on Hammer 21 (12,000 lb.), part DF3141 placed metal block tool in between die to dislodge part by tapping the hammer. On the second tap, flash fragments discharged from the forge. Employee was exposed to struck by hazard.
