29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about February 16, 2023, at 8975 SW  Shinnecock Drive Port St. Lucie, Fl, employees were exposed to a cave-in hazard while installing drainage pipes in an excavation approximately 12 feet 5 inches in depth that was not provided with an adequate protective system.
