29 CFR  1910.22(a)(2): The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition.  HA International, LLC located at 4243 South Avenue in Toledo, Ohio 43615. On or about February 21, 2023, the employer did not ensure that the floor of the dry stage set-up area was maintained in a clean condition. Employees were exposed to thermal burns through the handling of material that contains combustible dusts near the local ventilation system. The combustible dust pile was over 1 inch in thickness where employees perform staging operations. The employer failed to ensure adequate housekeeping for combustible dusts on the floors and working surfaces.

On or about February 21, 2023, the employer did not ensure that employees working near Mixer #8 were protected from fall hazards from heights greater than 4 feet (1.2 m). Employees are required to unload palletized material and pour into Mixer #8. The elevated platform edge protection was inoperable, exposing employees to falls from a height of approximately 8 feet.

29 CFR  1910.134(g)(1)(i)(A):The employer did not prohibit a tight-fitting facepiece to be worn by employees who had facial hair that comes between the sealing surface of the facepiece and the face:  On or about February 21, 2023, the employer did not prohibit a tight-fitting respirator to be worn by employees who had facial hair that impacts the sealing surface of the facepiece. Employees who work within the production area as process operators were required to wear 3M 7503 Half-Face respirators equipped with 3M 2091 P-100 filters and were observed to have facial hair that negatively impacts the seal of the respirator. Failure to ensure employees do not have facial hair that impacts the performance of the respirator exposes employees to inhalation hazards that can contribute to respirator disorders, asthma, and other adverse health effects associated with respirable contaminates.
