At the jobsite, located at 5180 113th Avenue North Clearwater, FL 33760; on or about 2/15/23,  the pressure body bar of the EEMCO PUE Rubber Mill machine, used by employees for the mechanical working of rubber and plastics compounds, had an inoperable alarm device to summon assistance in the event of a bite incident

The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor.  a. At the jobsite, located at 5180 113th Avenue North Clearwater, FL 33760; on or about 2/21/23, the rear emergency exit door was obstructed by polyurethane sheet carts, which exposes employees to fire and entrapment hazards.

At the jobsite, located at 5180 113th Avenue North Clearwater, FL 33760; on or about 2/15/23, employees operating the EEMCO PUE Rubber Mill machine, were exposed to caught-in between and struck-by hazards created by the exposed cutting blades, used to cut polyurethane sheet strips against the bottom roll while rotating.

At the jobsite, located at 5180 113th Avenue North Clearwater, FL 33760; on or about 2/21/23, employees were exposed to fire and electrical hazards in areas in front of electrical panels, in the manufacturing area near milling machines where employees move, work, and retrieve merchandise, in that access to electrical panels were blocked by polyurethane sheet carts.
