29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about February 22, 2023, one employee was performing roofing operations from a height of approximately 12 feet from the ground without using fall protection, exposing the employee to fall hazards.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. On or about February 22, 2023, one employee was performing roofing operations using a 24ft extension ladder that was not extended with its side rails at least three feet above the walking surface, exposing the employee to fall hazards.
