29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about February 16, 2023, at the job site located at 2122 East Birchwood Ave, Cudahy, WI, the employer did not develop, implement, and utilize machine specific procedures for the KSM-61 Core Shooter machine to control hazardous energy sources for when employees conduct servicing and maintenance activities. (b) On or about February 23, 2023, at the job site located at 2122 East Birchwood Ave, Cudahy, WI, the employer did not develop, implement, and utilize machine specific procedures for the KSM-62 Core Shooter machine to control hazardous energy sources for when employees conduct servicing and maintenance activities. (c) On or about February 23, 2023, at the job site located at 2122 East Birchwood Ave, Cudahy, WI, the employer did not develop, implement, and utilize machine specific procedures for the KSM-63 Core Shooter machine to control hazardous energy sources for when employees conduct servicing and maintenance activities.
