29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) 13525 County Road 8, FORT LUPTON, CO 80621: On or about March 27th, 2023, and at times prior, employees were exposed to falls hazards while placing  sealant on air-condition unit vents on top of trailer with no fall protection.

29 CFR  1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) 13525 County Road 8, FORT LUPTON, CO 80621: On or about February 21th, 2023, the employer failed to provide employees, voluntarily wearing filtering facepiece respirators, with the information contained in Appendix D regarding the proper use, maintenance, and limitations of respirators when trimming fiberglass.
