29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about February 23, 2023, an employee was exposed to a fall of approximately 23 feet while performing siding activities on a new residential home located at 36915 Stoney Ridge, North Ridgeville, Ohio 44039.  The employee was working on a ladder pick scaffold and the employer did not ensure he was using fall protection.

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about February 23, 2023, employees were exposed to a struck by hazard while performing siding activities on a new residential home located at 36915 Stoney Ridge, North Ridgeville, Ohio 44039.  employees were working above lower employees with tools and materials that could have fallen and the employer did not ensure the lower employees were wearing protective helmets.  Juan Alfredo Ventura DBA MVA Rooving was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a)(1), which was contained in OSHA inspection number 1515999, citation number 1, item number 1, and was affirmed as a final order on 3/30/21, with respect to a workplace located at 2063 Lanterman CR., Medina, Ohio 44233.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about February 23, 2023, employees were exposed to a fall of approximately 13 feet while performing siding activities on a new residential home located at 36915 Stoney Ridge, North Ridgeville, Ohio 44039.  The employees were working on all lower areas of the roof and the employer did not ensure they were using fall protection.  Juan Alfredo Ventura DBA MVA Rooving was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1515999, citation number 1, item number 2, and was affirmed as a final order on 3/30/21, with respect to a workplace located at 2063 Lanterman CR., Medina, Ohio 44233.
