29 CFR 1910.212(a): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Facility, 1000 Industrial Boulevard, Aliquippa, PA: On or about February 23, 2023 an employee's arm was caught in between the stationary bars and the UV box while reaching into the Largent #2 CNC Screen Printer while it was in operation to screen print mugs.

29 CFR 1910.147(c)(2)(ii):The employer's tagout energy control program procedures did not successfully comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection: a) Facility, 1000 Industrial Boulevard, Aliquippa, PA: On or about February 23, 2023, employees were setting up machines, making adjustments, changing screens and cleaning excess paint off inside of the CNC Screen Printers without locking out the energy sources.
