29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Facility, 1000 Industrial Boulevard, Aliquippa, PA: On or about April 8, 2025, an employee's hand was caught in between the moving carriage of the machine and the inside wall of the PeriOne Digital Printer while attempting to retrieve a piece of paper that fell out of a 16 ounce stainless steel tumbler she had placed inside for a logo to be printed on it. Moderne Glass was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1) , which was contained in OSHA inspection number 1652974, citation number 1, item number 1a and was affirmed as a final order on 5/24/23, with respect to a workplace located at 1000 Industrial Boulevard, Aliquippa, PA 15001.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a) Facility, 1000 Industrial Boulevard, Aliquippa, PA: On or about April 8, 2025, employees were exposed to potential electrical hazards while working at the PeriOne Digital printer because the electric panel in the vicinity had numerous items stored in front of it.
