29 CFR  1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)  Warehouse Area, 9901 Mountain View Drive, West Mifflin, PA 15122:  Observed on or about February 17, 2023, an electrical outlet box (110 volt) labeled 'N-35' and located below the a signed labeled 'Chips' was being used as a pendant style device.  The outlet box lacked proper strain relief and was equipped with knockouts.  The knockouts could be displaced by an employee's fingers during use.    b)  Warehouse Area, 9901 Mountain View Drive, West Mifflin, PA 15122:  Observed on or about February 17, 2023, an electrical outlet box (110 volt) labeled 'AS-2830' was being used as a pendant style device.  The outlet box lacked proper strain relief and was equipped with knockouts.  The knockouts could be displaced by an employee's fingers during use.    c)  Warehouse Area, 9901 Mountain View Drive, West Mifflin, PA 15122:  Observed on or about February 17, 2023, an electrical outlet box (110 volt) being used to power a Rentokil brand insect lamp and located near floor level near a Warehouse door in the Receiving Area was being used as a pendant style device.  The outlet box lacked proper strain relief and was equipped with knockouts.  The knockouts could be displaced by an employee's fingers during use.  Some of the knockouts were loose and the outlet box was damaged.

29 CFR  1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a)  Warehouse Area, 9901 Mountain View Drive, West Mifflin, PA 15122:  Observed on or about February 17, 2023, a flexible cord used to provide 110 volt power to the freezer alarm for a walk-in freezer lacked proper strain relief.  A section of the strain relief had separated from the cord near where the cord entered  freezer door.
