(b)   On or about February 28, 2023, at an establishment located at 3526 West Sherman Street, in Wausau, Wisconsin; the employer did not remove from service when the Toyota 571141 was unsafe due to large gouges and cracks in the tires.

The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:   On or about February 28, 2023, at an establishment located at 3526 West Sherman Street, in Wausau, Wisconsin; in the Bridge Bay, the P & H #5 overhead crane was not removed from service when the remote pendant was not working properly which interfered with proper operation in that employees had to manipulate the controls in order to utilize the east/west directions exposing employees to struck by hazards.

29 CFR  1910.254(d)(9)(iii):  Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions.  (a)  On or about February 28, 2023, at an establishment located at 3526 West Sherman Street, in Wausau, Wisconsin; in the Center Bay, the Ogden Dart 2101, S/N: 2156 12-30-09 was utilized with two Lincoln Arc Welders 287007 and 286020 used to weld various components had electrical tape wrapped around the cable due to damaged insulation exposing employees to electrical shock hazards.  (b)  On or about February 28, 2023, at an establishment located at 3526 West Sherman Street, in Wausau, Wisconsin; in the Center Bay, the Ogden Dart 2101, S/N: 2156 12-30-09 was utilized with two Lincoln Arc Welders 287007 and 286020 used to weld various components had exposed copper wires from the grounding leads exposing employees to electrical shock hazards.  (c) During the week of February 20, 2023 and thereafter, at an establishment located at 3526 West Sherman Street, in Wausau, Wisconsin; in the Bridge Bay, the Lincoln Squirt Welder #1 used to weld various components was rewired and exposing the wire and wrapped with electrical tape exposing employees to electrical shock hazards.

29 CFR  1910.334(a)(2)(i):  Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.  During the week of February 20, 2023 and thereafter, at an establishment located at 3526 West Sherman Street, in Wausau, Wisconsin; in Bridge Bay the Tornado vacuum used in conjunction with the Dart welding equipment had damaged outer insulation exposing employees to electrical shock hazards.
