29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: a) AAJ Construction Inc. On or about March 1, 2023 at the address of 3301 McLane Dr. Findlay, OH 45840: The employer did not ensure that employees operated from the floor of the aerial lift basket. Two employees utilizing aerial lifts to perform steel erection work activities (installing standing seam metal roof) stood on the basket rails as a means of increasing their working height. Employees operating in this condition were exposed to fall hazards of 18 feet or greater to the ground below. AAJ Construction Inc was previously cited for a violation of this Occupational Safety and Health Standard 1926.453(b)(2)(iv), which was contained in OSHA inspection number 1587432, citation number 1, item number 1 and was affirmed as a final order on August 9, 2022, with respect to a workplace located at 987 U.S. 1, Edison, NJ 08817.

On or about March 1, 2023 at the address of 3301 McLane Dr. Findlay, OH 45840: The employer did not ensure that aerial lift operator donned and secured their personal fall arrest systems before operating JLG 460SJ Telescopic Boom Lift (Aerial Lift). Employee utilizing aerial lift to perform steel erection work activities (installing standing seam metal roof) was exposed to falls of 18 feet or greater to the ground below. AAJ Construction Inc was previously cited for a violation of this Occupational Safety and Health Standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1587432, citation number 2, item number 1 and was affirmed as a final order on August 9, 2022, with respect to a workplace located at 987 U.S. 1, Edison, NJ 08817. Eight employees performing steel erection work activities (installing standing seam metal roof) were exposed to fall hazards of 18 feet or greater to the ground below.
