29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.  On or about March 2, 2023, KCB Masonry & Tuckpointing, LLC did not provide adequate means of access to a supported frame scaffold in a worksite located at 2715 South Compton Avenue in Saint Louis, Missouri. Employees were observed descending from a supported frame scaffold platform, that was approximately 15 feet high, by directly climbing the scaffold frames.

29 CFR  1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level.  On or about March 2, 2023, KCB Masonry & Tuckpointing, LLC did not protect employees from falling to a lower level, by utilizing fall protection, when they were tuckpointing from a supported frame scaffold at approximately 15 feet above a lower level in a worksite located at 2715 South Compton Avenue in Saint Louis, Missouri.

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about March 2, 2023, KCB Masonry & Tuckpointing, LLC did not protect an employee, working at a lower level on a supported frame scaffold, from falling bricks, tools, and/or debris. One employee did not have a protective helmet in a worksite located at 2715 South Compton Avenue in Saint Louis, Missouri.

29 CFR  1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects. When the falling objects were too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer did not place such potential falling objects away from the edge of the surface from which they could fall and did not secure those materials as necessary to prevent their falling.  On or about March 2, 2023, KCB Masonry & Tuckpointing, LLC did not protect employees from falling objects in addition to hardhats. Unsecured bricks were stacked on a ladder plank, and falling object protection such as toeboards, screens, guardrail systems, debris nets, catch platforms, or canopies was not used at a worksite located at 2715 South Compton Avenue in Saint Louis, Missouri.
