JOB SITE: 2278 South Jefferson Ave., St. Louis, MO. On or about 5/3/2023, the employer did not protect an employee, working at a lower level on a supported frame scaffold, from falling bricks, tools, and/or debris. One employee working at he base of the scaffold did not have a protective helmet.

29 CFR  1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  JOB SITE: 2278 South Jefferson Ave., St. Louis, MO. On or about 5/3/2023, the employer did not ensure that the working platform was fully planked and there was more than a 1 inch wide space between adjacent units and the uprights.  Workers were working on the scaffold at a height of approximately 24 feet.

JOB SITE: 2278 South Jefferson Ave., St. Louis, MO. On or about 5/3/2023, the employer did not ensure that the end of a platform greater than 10 feet in length extended over its support more than 18 inches. Employees were working on a platform that extended approximately 3 feet over it's support.

29 CFR  1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide.  JOB SITE: 2278 South Jefferson Ave., St. Louis, MO. On or about 5/3/2023, the employer did not install guys, ties and/or braces at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide. The employer did not install guys, ties and/or braces at all on a scaffold that was five (5) feet wide and over twenty-five (25) feet high.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.  JOB SITE: 2278 South Jefferson Ave., St. Louis, MO. On or about 5/4/2023, the employer did not provide adequate means of access to a supported frame scaffold at the job site. Employees were observed descending from a supported frame scaffold platform, that was over twenty-five (25) feet high, by directly climbing the inside and outside of the scaffold frames.

29 CFR  1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects. When the falling objects were too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer did not place such potential falling objects away from the edge of the surface from which they could fall and did not secure those materials as necessary to prevent their falling.  JOB SITE: 2278 South Jefferson Ave., St. Louis, MO. On or about 5/3/2023, the employer did not protect employees from falling objects in addition to hardhats. Unsecured bricks and tuckpointing materials were stacked on a ladder plank, and falling object protection such as toeboards, screens, guardrail systems, debris nets, catch platforms, or canopies was not used.

JOB SITE: 2278 South Jefferson Ave., St. Louis, MO. On or about 5/3/2023, the employer did not protect employees from falling to a lower level, by utilizing fall protection, when they were tuckpointing from a supported frame scaffold at approximately 24 feet above a lower level.
