29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) On a 5/12 pitched residential roof located at Elm Branch Road, Lot 328 in Saint Johns Florida: On or about March 02, 2022, the employer exposed employees to fall hazards ranging from 6 to 22-foot, in that fall protection was not utilized by employees, while performing roofing / sheathing work.  Melchor Remodeling Construction Inc was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1628192 Citation number 1, Item number 2 and was affirmed as a final order on 1/26/23 with respect to a workplace located at Lot# 18, Back Creek Dr., Saint Augustine Florida 32092.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a.  On March 17, 2023, on a steep pitched (5/12) residential roof at the jobsite located at Lot #58, on Elm Branch Road Mill Creek Forest Subdivision, Saint Johns Florida 32259: The employer is exposing employees to eye or face hazards from flying particles, specifically; employees were working with pneumatic nail guns without the use of eye or face protection.  Melchor Remodeling Construction Inc was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1628192 Citation number 1, Item number 1 and was affirmed as a final order on 1/26/23 with respect to a workplace located at Lot #18, Back Creek Drive in Saint Augustine, Florida 32092.
