29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a.  On the roof of a temporary residential residence located at 95191 Sandy Pointe Drive, Lot 74, Fernandina Beach, Florida 32034: On or about April 12, 2023, the employer exposed an employee to eye or face hazards from flying particles, in that the employee was operating a pneumatic nail gun without the use of eye protection.  Melchor Remodeling Construction Inc was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1628192 Citation number 1, Item number 1 and was affirmed as a final order on 1/26/23 with respect to a workplace located at Lot #18, Back Creek Drive in Saint Augustine, Florida 32092.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. At a temporary residential job site located at 95191 Sandy Pointe Drive, Lot 74, Fernandina Beach, FL 32034.  On or about April 12, 2023,  on a 8:12 pitched roof, the employer exposed an employee to fall hazards ranging from 10 to 29-feet, in that fall protection was not utilized by the employee, while decking the roof.   Melchor Remodeling Construction Inc was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1628192 Citation number 1, Item number 2 and was affirmed as a final order on 1/26/23 with respect to a workplace located at Lot# 18, Back Creek Dr., Saint Augustine Florida 32092.

Inside home, at a temporary residential worksite located at 95191 Sandy Pointe Drive  - Lot 74, Fernandina Beach, FL 32034: On or about April 12,  2023, the employer exposed employees to fall hazards, in that, the employees used the top steps of  Werner 6-foot A-frame ladders to reach the rafters, exposing themselves to fall hazards.  b. Inside home, at a temporary residential worksite located at 95191 Sandy Pointe Drive - Lot 74, Fernandina Beach, FL 32034: On or about April 12, 2023, the employer exposed employees to fall hazards, in that, employees used the top steps of a Werner 4-foot-a-Frame ladders to reach the rafters, exposing himself to fall hazards.
