29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  (a)Chable Roofing & Construction Inc. at 705 North Bahnson Avenue, Sioux Falls, SD 57103: On or about and at times prior to March 2, 2023, the employer did not ensure frequent and regular inspections of the job site were  conducted.  Employees were conducting roofing operations on a residential property and were not utilizing  fall protection systems as well as ladder hazards and lack of training provided to employees.  This condition exposed employees to fall hazards greater than 6 feet.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  (a)Chable Roofing & Construction Inc. at 705 North Bahnson Avenue, Sioux Falls, SD 57103: On or about and at times prior to March 3, 2023, the employer did not ensure that employees were utilizing appropriate eye protection while performing framing operations with pneumatic nail guns.  This condition exposed employees to eye injuries including the introduction of foreign particles into the eye while working.

29 CFR  1926.1053(b)(1):  The employer did not ensure portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)Chable Roofing & Construction Inc. at 705 North Bahnson Avenue, Sioux Falls, SD 57103: On or about and at times prior to March 2, 2023, the employer did not ensure that portable ladders being used for access to the roof were extended at least three feet above the upper landing surface of the residential roof. This condition exposed employees to fall hazards associated with ladder access.
