This resulted in hospitalization and amputation. This was documented on or about 3/6/2023.

At the worksite located at 2000 Forest Ave. Kansas City MO, employees were exposed to point of operation hazards when weekly inspections were not conducted on the Toledo 57 press to determine the condition of the clutch/brake mechanism. This resulted in hospitalization and amputation. This was documented on or about 3/6/2023.
