29 CFR  1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions.   On or about March 7, 2023, employees installed utility/drainage pipes on the site's northeast side in an excavation that was not inspected throughout the shift for evidence of a situation that could have resulted in possible cave-ins. If inspected by a competent person, they would have discovered that the excavation did not have a means of egress or cave-in protection in class "c" soil in an excavation deeper than five (5) feet or that a means of egress was not present in the excavation that was deeper than four (4) feet.

29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.  On or about March 7, 2023, employees installed utility pipes on the site's northeast side in an excavation where the employer did not instruct each employee in the recognition, avoidance, control, or elimination of unsafe conditions and the regulations applicable to their environment:  a.	Employees, installing a drainage pipe in an excavation, were not trained to recognize the hazards of working in a trench.  South La Contractors LLC (DBA RedStag) was previously cited for a violation of this occupational safety, and health standard or its equivalent standard 1926.651(c)(2), which was contained in OSHA inspection number 1609661, citation number 1, item number 3-b, and was affirmed as a final order on March 16, 2023, with respect to a workplace located at 40070 Parker Rd, PRAIRIEVILLE, LA 70769.

On or about March 7, 2023, employees installed utility pipes on the site's northeast side in an excavation four (4) feet or more in-depth without a safe means of egress.  South La Contractors LLC (DBA RedStag) was previously cited for a violation of this occupational safety, and health standard or its equivalent standard 1926.651(c)(2), which was contained in OSHA inspection number 1609661, citation number 1, item number 3-a, and was affirmed as a final order on March 16, 2023, with respect to a workplace located at 40070 Parker Rd, PRAIRIEVILLE, LA 70769.

On or about March 7, 2023, employees were installing utility pipes that were not protected from excavated spoils or other materials or equipment that could pose a hazard by falling or rolling into excavations. The spoils and equipment were not two feet or more from the edge of the excavation.   South La Contractors LLC (DBA RedStag) was previously cited for a violation of this occupational safety, and health standard or its equivalent standard 1926.651(c)(2), which was contained in OSHA inspection number 1609661, citation number 1, item number 2, and was affirmed as a final order on March 16, 2023, with respect to a workplace located at 40070 Parker Rd, PRAIRIEVILLE, LA 70769.
