29 CFR 1926.501(b)(7)(ii):Each employee at the edge of a well, pit, shaft, and similar excavation 6 feet (1.8 m) or more in depth shall be protected from falling by guardrail systems, fences, barricades, or covers. On or about March 15, 2023, workers installed drainage piping on the site's south side in an excavation four (4) feet or without barriers, guardrails or personal fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about March 15, 2023, on the south side of the project site, employees performing shingle installation was utilizing a Werner twenty-four (24) foot aluminum ladder to access a sloped roof without the side rails extending at least 3 feet above the roof's landing.
