29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) 200 Maple Ave., Monsey, NY (New Residential Construction): Employees working on the ground level were exposed to a struck by hazard from falling objects, and were not wearing protective helmets. The employer did not ensure that employees were provided with protective helmets, on or about 3/07/23.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) 200 Maple Ave., Monsey, NY (New Residential Construction): An employee engaged in framing work, was using a handheld pneumatic nail gun, without eye and face protection. The employer did not ensure that employees were wearing eye and face protection, thus exposing employees to an eye and face hazard, on or about 3/07/2023.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) 200 Maple Ave., Monsey, NY (New Residential Construction): Employees were required to conduct framing operations and were exposed to a fall hazard of over 12 feet above the lower surface, without the use of a personal fall arrest system or other means of fall protection. The employer did not ensure that employees were provided with a personal fall arrest system, or other means of fall protection, thus exposing employees to a fall hazard, on or about 3/07/23.
