29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Worksite was located at 355 Martin Luther King Jr. Way, Bridgeton, NJ 08302  The employer did not develop and implement an energy control program consisting of energy control procedures, employee training and periodic inspections for employees who worked with production machines including, but not limited to,  E.C. engineering S.r.l. cone machine, Model number STK600V2 and Serial number 16140009. An employee suffered an arm amputation after the machine activated while the employee was reaching into it   to remove stuck cones.   Violation occurred on or about January 11, 2023.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) Work site was located at 355 Martin Luther King Jr. Way, Bridgeton, NJ 08302  Workers were exposed to struck by hazards as the company had a forklift operator operating the Raymond Model 750 - DR32TT; Serial Number 750 - 15 - BC49477 forklift throughout the facility who was not trained to be operating forklifts.  Violation occurred on or about February 9, 2023.

Violation occurred on or about January 11, 2023.
