29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) 2001 Mission Drive, New Cumberland, PA 17070 - East Bulk - General Purpose Warehouse Bldg. 82 - Battery Charging Area (Bay 3): On or about March 9, 2023, employees who utilize hazardous chemicals (electric forklift battery charging) were not provided with suitable facilities for quick drenching or flushing of the eyes and body; the emergency eyewash was signed off as inspected on March 8, 2023 but was covered in dust and dead insects. b) 2001 Mission Drive, New Cumberland, PA 17070 - East Bulk - General Purpose Warehouse Bldg. 83 - Battery Charging Area: On or about March 9, 2023, employees who utilize hazardous chemicals (electric forklift battery charging) were not provided with suitable facilities for quick drenching or flushing of the eyes and body; the emergency eyewash was last inspected in March 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Distribution Process Workers conducting routine warehouse operations are exposed to lead associated with paint on wall surfaces that have experience damage from material handling equipment on or about March 9, 2023. b.) 2001 Mission Drive New Cumberland, PA 17070 - East Bulk - General Purpose Warehouse Bldg. 52 - Distribution Process Workers conducting routine warehouse operations are exposed to wall surfaces containing chipping lead-based paint, on or about March 9, 2023. c.) 2001 Mission Drive New Cumberland, PA 17070 - DLA Susquehanna - The employer did not establish a current Occupational Safety and Health Program as required by DoDI 6055.1, Enclosure 2,(3)(b), on or about March 15, 2023. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
