Following Equipment/Non Functional Devices: a) Komatsu - Model: FG25T-12, Serial: 554600A (nonoperational horn and right hand brake light) b) Komatsu - Model: FG25T-12, Serial: 554601A (nonoperational horn)  Casino Pauma was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(1), which was contained in OSHA inspection number 1347334, citation number 1, item number 1 and was affirmed as a final order on November 20, 2018, with respect to a workplace located at 777 Pauma Reservation Rd, Pauma Valley, CA 92061.
