29 CFR 1926.304(i)(1): Each circular hand-fed rip saw was not guarded by a hood that completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut. (a) On March 7, 2023, at 549 Aviamar Circle Grand Forks North Dakota, employees using a Dewalt portable table saw (Model Number DWE7490) to rip sheets of plywood, the saw was not equipped with a guard, the employees were exposed to hazards associated with contacting the cutting blade of a saw.

(a) On March 7, 2023, at 549 Aviamar Circle Grand Forks, North Dakota, the employer did not inspect an extensible boom forklift (GEHL DL 10H) prior to using it to elevate an employee on a platform to heights greater than 10 feet. The forklift tine attachment was not fully secured to the boom. The mechanism for locking the attachment pins in place was not fully engaged allowing for the tine assembly to be unintentionally released from the forklift.
