29 CFR 1926.304(i)(1): Each circular hand-fed rip saw was not guarded by a hood that completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut.  (a) On March 7, 2023, at 549 Aviamar Circle Grand Forks North Dakota, employees using a Dewalt portable table saw (Model Number DWE7490) to rip sheets of plywood, the saw was not equipped with a guard, the employees were exposed to hazards associated with contacting the cutting blade of a saw.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On March 7, 2023, at 549 Aviamar Circle Grand Forks, North Dakota, employees were exposed to fall hazards greater than 13 feet while applying plywood to roof trusses with a pneumatic Bostich roofing nailer without the use of fall protection.

29 CFR  1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.   (a) On March 7, 2023, at 549 Aviamar Circle Grand Forks, North Dakota, the employer did not inspect an extensible boom forklift (GEHL DL 10H) prior to using it to elevate an employee on a platform to heights greater than 10 feet.  The forklift tine attachment was not fully secured to the boom. The mechanism for locking the attachment pins in place was not fully engaged allowing for the tine assembly to be unintentionally released from the forklift.
