29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about March 16, 2023, the employer did not ensure methods of machine guarding were provided, exposing employees working in and around spindle wheels to possible injury from the nip point created by the wire traversing over the wheel. Spindle wheel - Progressive spooling machine model MS630 -"PMC" (OSC-2), Progressive spooling machine model 630 - "PMC" (OSC-5) b) On or about March 16, 2023, the employer did not ensure methods of machine guarding were provided, exposing employees working in and around the rotating spool to possible injury from being caught in the material accumulating around the wheel. Rotating spool - Progressive spooling machine model MS630 -"PMC" (OSC-2), Progressive spooling machine model 630 - "PMC" (OSC-5) c) On or about March 16, 2023, the employer did not ensure methods of machine guarding were provided employees are at risk of being caught in material accumulating around the scrap wheel. Scrap wheels - Progressive spooling machine model 630 - LH (OSC-1), Progressive spooling machine model MS630 -"PMC" (OSC-2). d) On or about March 16, 2023, the employer did not ensure methods of machine guarding were provided exposing employees working in and around material feed roll to possible injury due to caught in hazards associated with rotating shaft.
