29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods necessary for energy isolation and control: a) On or about March 16, 2023, at the packaging 1 and 2 machines, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or to equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. Machine operators are exposed to broken bones or amputations when using hand tools to unjam paper from the PPP press or cleaning the machine dies.
