29 CFR 1910.109(b)(1): Explosives or blasting agents were stored, handled, and/or transported when such storage, handling, and transporting of such explosives or blasting agents constituted an undue hazard to life: (a) Covanta Lee County, Fort Myers, FL - On or about March 15, 2023, employees performing on-line de-slagging/blasting services of Economizer #2 were exposed to an explosion hazard, in that, the blaster in charge was setting the detonation cord by hand.

29 CFR 1910.109(e)(4)(viii): Leading wires did not remain shorted and were connected to the blasting machine or other source of current prior to the charge being fired. (a) Covanta Lee County, Fort Myers, FL - On or about March 15, 2023, employees performing on-line de-slagging/blasting services of Economizer #2 were exposed to an explosion hazard, in that, the employer did not ensure the leading wires were shorted and the alligator clips were attached prior to the charge being placed in the boiler.
