Violation occurred on or about 8/29/2024.

Before a blast is fired, the employer did not require that a loud warning signal be given by the person in charge, who did not make certain that all surplus explosives were in a safe place, all persons and vehicles were at a safe distance or under sufficient cover, and that an adequate warning was given. This condition exposed employees to permanent disabling injuries. Violation occurred on or about 8/29/2024.
